CMC is excited to announce we are moving to paperless! MyChart now gives us the ability to send all well visit (physical forms), after visit summaries, immunization records, request medical records, billing and payment information directly through the portal. We encourage everyone to make sure that you are MyChart active as all annual form updates (done during E-check in) and most well visit questionnaires for the office are now on MyChart. CMC will no longer print physical forms or after visit summaries unless requested as these are in your MyChart once your visit is complete. We will continue to give an updated immunization record. We also ask all patients to transition to a paperless billing status to view and pay your accounts online. This allows CMC to reduce many issues with address changes, mail problems and allows for refunds directly to the credit card you paid on. CMC appreciates your participation as this is a big administrative savings for the practice.
