To All Former And Current Patients of Children’s Medical Center:
Children’s Medical Center is in the Process of Medical Record Expunging.
If you are/were a patient between the years of 2000 to 2010 and would like
to obtain your paper medical record, please give us a call. We will be reaching out
to contact as many patients as we can before the records will be destroyed, but may
not have current contact information. After 30 days all records that are eligible for destruction,
will be destroyed. Children’s Medical Center will no longer have access to your
prior records except Immunizations that are currently up to date in our electronic system.

We will need the child and parents full names and the child’s date of birth.
