DO YOU KNOW HOW TO GET YOUR SCHOOL AND CAMP FORMS AT CMC?
1) You will receive a printed and signed copy of your child’s Physical and Immunization Form at the completion of his/her well visit (so please hang on to these).
2) CMC is using MyChart for all replacement Physical and Immunization Form requests. Below is a sample of where to locate your child’s information in your MyChart account. The CMC staff can no longer provide replacement copies of lost forms for you so please activate your child’s MyChart account today if you have not yet done so.
