- Before requesting a form to be filled out, please fill out as much as you possibly can on the form. This includes your child’s information, known medical conditions, and medication name/dosage/when to give it. We will double check everything with your child’s medical record, but please be as specific as you can, especially with medications you want them to have.
- Upload the form to MyChart by selecting Messages -> Ask a medical question -> Request form/letter -> then attach form. Importantly, we cannot accept screenshots or pictures of forms; you must upload the form in its original format.
Please request your form a few weeks before it is due. We will do our best to return all form requests within 72 business hours, but this is not always possible.
