Happy New Year to all of you! With the start of a new year, we are required to update all of our patient demographic and policy forms. These include the Patient Information Packet, HIPAA Policy, Financial Policy, and No Show Policy. All of these forms are located under the Forms page (found under Quick Links on the Home Page or under the Appointments tab). Please print and complete these forms at home and bring to your child’s appointment.
We also would like you to activate your My Chart account. My Chart allows you to email your child’s doctor, view recent lab tests, view the growth chart, and download immunization and health appraisal forms. Talk to our front desk staff to activate your account and visit our My Chart page.
Finally, if you have not done so already, submit your email address on the Home Page to receive these News Blogs so you can stay abreast of important messaging from the office.
